Join Us in Our Mission to End HIV in CT
The Connecticut HIV Planning Consortium (CHPC) is dedicated to establishing a coordinated, statewide HIV care and prevention system that actively reduces new infections and connects those affected by HIV/AIDS to essential services. CHPC members come from diverse backgrounds and bring a wealth of expertise to our collaborative planning efforts. Together, we work to reflect and address Connecticut’s unique HIV/AIDS epidemic across all age groups, races, ethnicities, genders, sexual orientations, and risk factors.
Who Can Join?
The CHPC aims to maintain a membership of 30-35 individuals from various stakeholder groups, including healthcare providers, community advocates, public health experts, and individuals with lived experience. CHPC membership is open to individuals who are committed to our mission and bring a valuable perspective to our HIV prevention and care planning efforts.
Application Process
- Complete the Application
Fill out the CHPC Membership Application, ensuring all sections are completed to provide a comprehensive overview of your background and experience. - Email Your Application
Send your completed application to Ken Plourd at plourd@xsector.com. - Follow-Up
Once we receive your application, a CHPC representative will reach out to discuss the next steps and answer any questions about joining the consortium.
